What services do you offer at your family medicine practice?
Our practice offers a wide range of services including preventive care, wellness check-ups, management of chronic conditions like diabetes and hypertension, acute care for short-term illnesses, vaccinations, routine screenings, and minor surgical procedures. We also provide tailored care for children, adults, and the elderly.
How can I schedule an appointment?
Appointments can be scheduled by calling our office during working hours, through our online patient portal, or via email. We also offer a limited number of same-day appointments for urgent needs.
What should I bring to my first appointment?
Please bring a valid ID, your insurance card, a list of any medications you are currently taking (including over-the-counter medications and supplements), your medical history, and any previous medical records you can provide.
Do you offer telehealth or virtual consultations?
Yes, we offer telehealth services. These virtual consultations can be scheduled just like in-person visits and are conducted via a secure video conferencing platform. You will need a computer or device with a camera and internet access.
What are your office hours?
Our office is open Monday to Friday from 8 am to 6 pm. We are closed on weekends and public holidays. Emergency contacts are provided for after-hours care.
How do you handle medical emergencies?
In the event of a medical emergency, please call 911 or go to the nearest emergency room. For urgent but non-life-threatening issues outside of our office hours, you can call our after-hours number for guidance.
Can I get lab work done at your practice?
We offer a variety of in-house lab services for your convenience. For more specialized tests, we partner with local accredited labs.
Do you provide pediatric care?
Yes, our practice provides comprehensive pediatric care including well-child checkups, immunizations, growth and development monitoring, and treatment for acute illnesses.
How do you manage chronic illnesses?
Our approach to managing chronic illnesses involves a personalized treatment plan, regular monitoring, lifestyle and diet recommendations, medication management, and coordination with specialists when necessary.
What insurance plans do you accept?
We accept most major insurance plans. Please contact our office for a specific list of providers. For uninsured patients, we offer a sliding scale fee structure.
How do I refill a prescription?
Prescription refills can be requested through our patient portal, by calling our office, or by contacting your pharmacy. Please allow 48 hours for prescription refills.
Do you offer women’s health services?
Our women’s health services include routine gynecological exams, breast exams, Pap smears, contraception counseling, and menopause management.
How do I access my medical records?
Patients can access their medical records through our secure online patient portal. Copies of medical records can also be requested at our office.
What should I do if I need to cancel or reschedule an appointment?
Please notify us at least 24 hours in advance if you need to cancel or reschedule your appointment. Late cancellations or missed appointments may incur a fee.
Can I get vaccinations at your practice?
We offer a full range of vaccinations for both children and adults, including seasonal flu shots, travel vaccinations, and routine immunizations.
How do you coordinate care with specialists?
We coordinate care with specialists through referrals, sharing of medical records, and regular communication to ensure continuity and comprehensiveness of care.
What languages are spoken at your practice?
Our staff are fluent in English and Spanish. We also offer interpretation services for other languages upon request.
What should I expect during a wellness check-up?
A wellness check-up typically includes a physical examination, discussion about your lifestyle and habits, routine screenings based on your age and gender, and vaccinations if needed.
How can I provide feedback or file a complaint?
We value your feedback. You can provide feedback directly to our staff, through our patient satisfaction surveys, or by contacting our office manager. Formal complaints can also be submitted in writing to our office.
